In an attempt to help speed up the registration process for our parents, we are providing some of the necessary forms online.

You may open each of the forms listed below one at a time.  Then type into each field on screen, providing the appropriate information, use the tab key to advance to the next field, and then print out the form to bring with you to registration on August 8th or 9th.

Adobe Reader 5.0 or above will be necessary to use each of the forms listed below.  If you need to download the Reader, you can do so by clicking the image below.

Remember, the forms with your answers can only be printed out.  Your answers cannot be saved to your local drive unless you have the full version of Adobe Acrobat.
